Who can refer

We accept referrals from GPs, consultants, nurse specialists, therapists and social workers based in the local community or at Guy's and St Thomas'.

How to refer

Some of our clinics are also available through Choose and Book. 

We will book the patient into the most appropriate clinic therefore the more detail we have from the referral is helpful. For some clinics we'll contact the patient before we offer an appointment to make sure they're seen at the most appropriate clinic.

Urgent referrals

Patients can be referred by primary care staff working in Southwark and Lambeth using the Telephone Advice Line (TALK):

  • 0207 188 1465, 24 hours a day, 365 days per year

TALK is part of the Southwark and Lambeth Integrated Care Programme and provides access to specialist advice from a consultant geriatrician.

Healthcare colleagues within Guy's and St Thomas' NHS Foundation Trust can refer via EPR using the HOT clinic referral options, with as many details as possible to explain why this would be considered an urgent referral.

We do not take emergency referrals.
