Routine referrals

Referral criteria
Lymphoedema referrals are currently accepted from GPs or other health professionals for patients who:

are over the age of 18; and
-
have received all, or part of their treatment for cancer within Guy's and St Thomas'.
Referrals from King's College Hospital are accepted where patients require assessment for swelling secondary to treatment for breast cancer, where their care is under the NHS.

Making a referral
To refer a patient, please complete the lymphoedema referral form (Word 31Kb) and post or fax it to the lymphoedema team using the details at the top of the page.
Any other patients with lymphoedema secondary to treatment for cancer who do not meet with the above requirements will have to have an Exceptional Treatment Arrangement Form completed by their GP.
Urgent referrals
Urgent referrals are accepted for patients with lymphhoedema, or who require palliative management. Please complete the lymphoedema referral form as above.
Non-cancer referrals
Oedema, or lymphoedema, resulting from causes other than cancer are seen by the vascular team| at St Thomas'.
