Hospital and/ or NHS number

Clinical information ie disorder and severity
pedigree (if relevant/ useful)
Tests requested/ required
Specimen collection:
Sample required: EDTA (purple top tube)
5-10ml venous blood should be collected from a peripheral vein into tubes containing EDTA as anticoagulant, and the tubes inverted 8-10 times, WITHOUT SHAKING, to ensure proper anticoagulation. Whole blood collected into sealed vacuum tubes can be stored at room temperature for 24-48 hours without detectable deterioration of the sample.
