Lambeth Community Diabetes Service nominated for a Care Integration Award

The Diabetes Modernisation Initiative and Lambeth Community Diabetes Service, provided by the Lambeth Diabetes Intermediate Care Team, have been nominated for a Care Integration Award.

The award recognises the team's excellent work to provide integrated diabetes care in the Lambeth community.

The team provides a whole range of services to local residents, including multi-disciplinary, specialist clinics in three locations around the borough, structured education, access to insulin start groups, outreach work for GP practices in the borough, virtual clinics with GP practices and more.

Patient feedback on the services has been extremely positive, and the team is making a real difference to the health of people with diabetes in Lambeth. The nomination is recognition of this achievement, and our fingers are crossed for success at the awards ceremony in July!
