Referrals

For all programmes, please make sure your referral includes all the information we need. We will reject referrals which do not have all the information needed.

South east London healthy living programme

We accept referrals from GPs, practice nurses and other health professionals.

We accept referrals for people who:

We do not accept referrals for people who:

  • are pregnant or planning pregnancy
  • are receiving palliative or end of life care
  • have had a myocardial infarction or stroke within the last 3 months
  • have blood pressure readings of >180 mmHg systolic OR >120 mmHg diastolic taken within the last 6 months (consider re-referral when blood pressure is optimised)

GP and practice nurses referrals

All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.

You need to select:

  • 'Dietetics' as specialty
  • 'Weight Management' as clinic type
  • 'SEL Tier 3 Healthy Weight Programme' as service name

The referral form is on EMIS or DXS. It is called 'SEL Healthy Weight Management Programme Referral form'.

Other health professionals

Body mass index (BMI) requirements

If your patient has no co-morbidities they must have a minimum BMI of 40 if they're white or 37.5 or above if they're of another ethnicity.

If your patient has type 2 diabetes or at least 2 co-morbidities associated with obesity they must have a minimum BMI of 35 if they're white or 32.5 if they're of another ethnicity.

If your patient has been diagnosed with type 2 diabetes within the last 2 years they must have a minimum BMI of 30 if they're white or 27.5 if they're of another ethnicity.

Obesity co-morbidities are:

  • hypertension
  • idiopathic intracranial hypertension
  • NAFLD, NASH or other hepatic steatosis
  • obstructive sleep apnoea or obesity hypoventilation syndrome
  • prediabetes
  • previous stroke or myocardial infarction

Southwark healthy weight programme

We accept referrals from GPs, practice nurses and other health professionals.

We accept referrals for people who:

  • are over 18
  • are living in Southwark or registered with a Southwark GP
  • have a body mass index (BMI) above 30
  • are of Black, Black African or Black Caribbean ethnicity with a BMI over 27.5

The referral form is available on EMIS or DXS. It is called 'single point of access tier 2 WM / HLH'.

Lambeth healthy weight hub

We accept referrals from GPs, practice nurses and other health professionals.

Patients can also self-refer using the Lambeth healthy weight hub self-referral form.

We accept referrals for people who:

  • are aged 18 or over
  • are registered with a GP in Lambeth or are a Lambeth resident
  • are able to speak and read English
  • are motivated to change lifestyle behaviours
  • have a body mass index (BMI) between 30 and 40
  • are of Black, Black African, Black Caribbean, LatinX, South Asian (including mixed ethnicities) ethnicity with a BMI between 27.5 and 40

Up!Up!

We accept referrals from GPs, practice nurses and other health professionals.

Patients can also self-refer using the Up!Up! self-referral form.

We accept referrals for people who:

  • are over 18
  • are registered with a GP in Lewisham or a Lewisham resident
  • have a body mass index (BMI) above 30
  • are of Black, Black African or Black Caribbean ethnicity with a BMI over 27.5
  • are women with a waist circumference over 80 centimetres (32 inches)
  • are men with a waist circumference over 94 cm (37 inches)

Exclusion criteria

We do not accept referrals for people who:

  • are pregnant or planning pregnancy
  • are receiving palliative or end of life care
  • have had a myocardial infarction or stroke within the last 3 months
  • have blood pressure readings of >180 mmHg systolic OR >120 mmHg diastolic taken within the last 6 months (consider re-referral when blood pressure is optimised)
  • have a clinically diagnosed eating disorder
  • cannot take part in a group intervention
