Working with homeless patients attending or admitted to hospital

Contact details

Opening times: 9am-5pm Monday-Friday. Please note referrals made after 3pm will not be seen until the next business day.

How do I get an appointment?

You must be referred to us by a healthcare professional from the department of either Guy's or St Thomas', or King's College hospital where you are visiting or receiving treatment. You can also be referred by community health services such as the health inclusion team.

What to expect at your appointment

If you are referred to our service, one of our team will with you and conduct an in depth assessment of your situation, including geographical and housing history. Once this assessment has taken place and any necessary verification sought, the team member will be able to advise and if appropriate assist with your housing issues.
