About the weight management programme

Weight management programme for Southwark residents

You can be referred to the programme by your GP. Please see your GP if you would like to be referred. Please be advised this programme is for Southwark residents only (or patients registered with a Southwark GP).

Once you have been referred by your GP you will be contacted by our referrals team if you have been accepted on to the programme.

Am I suitable for the programme?

Both the programmes (tier 2 and tier 3) are suitable for patients who are:

  • over 18
  • motivated to lose weight
  • willing to take part in a group based programme requiring regular attendance.

About the programmes

The nutrition education content of both programmes is similar. The key difference between the programmes are that those completing the tier 2 programme attend 12 weekly sessions, whereas those completing the tier 3 programme are seen over a period of a year.

  • Tier 2 programme

    This programme comprises of:

    • a 12-week programme including a 1-1 initial assessment with a physical activity specialist followed by 11 weekly exercise and nutrition education sessions
    • follow up appointments at six, 12 and 24 months.

    Eligibility criteria

    • Age 18 or over
    • BMI ≥ 30 (or of Asian origin with a BMI ≥ 27.5)
  • Tier 3 programme

    This programme comprises of:

    • a 1-1 initial assessment with a dietitian
    • 11 group sessions over the course of a 12 month period
    • patients will be reviewed by a team made up a range of different healthcare professionals
    • follow up appointments at six, 12, 18 and 24 months
    • support via the OVIVA app. This app provides support between sessions. It features a facilitated and moderated group chat function which enables the dietitian to respond to questions, to correct misinformation, provide motivation and set group challenges. The dietitian is able to identify clients who may be struggling and offer coaching.

    Eligibility criteria

    • Age 18 or over
    • BMI ≥ 40 (or of Asian origin with a BMI ≥ 37.5)
    • Should have had a weight loss intervention at Tier 2 and/or tried commercial programmes
