Your appointment
You must be a resident in Southwark or Lambeth to see our adult community nursing team. The service is offered primarily to people who are housebound and unable to access nursing care from their GP practice or elsewhere.
You will see a community nurse who may have a student with them. They will be wearing uniform, and will clearly identify themselves as a Guy's and St Thomas' community nurse. They will discuss a care and treatment plan based upon an assessment of your nursing care needs.
