Your @home team will include:
- a @home GPs who provide clinical support and works with your own GP
- a specialised @home matron who will coordinate your care and works closely with the @home GP.
The team also includes nurses, therapists, pharmacists and social workers, who will visit you in your home and provide the care that you need.
