The integrated heart failure team is made up of staff from Guy’s and St Thomas’ and King’s College Hospital. Each locality in Southwark and Lambeth has their own dedicated heart failure specialist community nurse and consultant cardiologist with two pharmacists who work across Southwark and Lambeth.
