Bringing hospital care to your home

We provide community health services across Lambeth and Southwark, allowing us to deliver seamless care for our patients.

We work in partnership with colleagues within the health economy and with the local authorities and voluntary/community groups to provide holistic care.

The range of services we provide

We deliver services in a variety of locations, including in GP practices, health centres, schools, community buildings and in patients' homes.

Southwark and Lambeth Integrated Care (SLIC)

SLIC is the partnership between the three local NHS Foundation Hospital Trusts (Guy’s and St Thomas’, South London & Maudsley and King’s College Hospital) and local GPs, clinical commissioning groups, local authorities, and others, supported by Guy’s and St Thomas’ Charity.
