To refer a patient
The information on this page explains how to refer a patient to our service. You can specify a referral to our hospital or community teams.

Referral criteria
Patients with serious life-limiting or life-threatening illness who require support with:
- pain/symptom management
- advance care planning
- meeting the psycho-social needs of the patient and their family and/or significant other
- care at the end of life.
Patients will be discharged from our service if the above needs are addressed and our input is no longer required.

How to refer
To refer to our teams please complete our palliative care referral form (PDF 346Kb) in full, with relevant supporting information.
