To make a referral to our radiology service, please complete a referral form:

The following tests require an appointment:
- barium swallow and enema scans
- breast ultrasound (referrals need to be sent directly to the breast unit)
- CT
- MRI (cardiac MRI requests must be sent to cardiology)
- ultrasound
- ultrasound uterine embolisations.

Urgent / two week wait referrals
Please email these to gst-tr.radiologyreferrals@nhs.net, and please indicate clearly that the request is urgent.
Referral forms
Clinical details must be legible and the examination requested must be clearly printed.
Information required
- To avoid unnecessary delays for the patient, please ensure that request forms clearly show the patient's name, date of birth, address and contact number.
- For relevant patients confirmation is required of patient pregnancy status.
- Up to date blood test results must be provided for all CT and MRI requests as well as ultrasound biopsy.
- For MRI referrals details of any metal implants or pacemakers must be included.
- Overseas Patients must be clearly identified.
