About us
The social work department helps with discharge planning for elderly and disabled patients into the community, expectant mothers booked into St Thomas' and children in our hospitals.
We have three borough council social services teams:
- Lambeth older people and disability team
- Southwark hospital discharge team north
Referrals
Referrals are made by staff working on our hospitals or community service teams. We do not accept referrals from GPs.
