What can social workers do?
We offer advice and support to patients, their family and carers. This can include care home placements, advice on accommodation or benefits you may be entitled to and assessments for community care services, such as meals on wheels and domestic support.
The care you can expect
If your nurse thinks you need a care package for when you leave hospital (known as being discharged from hospital), he/she will refer you to the social work services team.
A social worker will see you on the ward a couple of days before your planned discharge date. He/she will work with the team caring for you and your relatives/friends if needed.
