We accept referrals from GPs, district nurses and nurse practitioners for patients living, or registered to a GP in Lambeth and Southwark via the tissue viability referral email address. Please complete the referral form and attach.

Hospital referrals should also by made to gst-tr.tissueviabilityservicereferrals@nhs.net.

How to make a community referral

To make a referral, download the relevant form and return it to the contact details provided on the form:
