Routine referrals
We accept referrals to the programme from GPs only. Please note your patient must be registered with a GP in Southwark.
If you have been asked by your patient to refer them to the weight management programme, please download the form titled 'Dietetics – weight management – referral' and named 'Weight management pilot referral form SOW1435'.
If you do not have access to DXS, please complete the referral form (Word 86Kb) and email to gst-tr.referralsleips@nhs.net.
