Who can refer?
We welcome referrals from primary care and other hospitals. Several local adult cardiologists have an interest in ACHD and many patients are managed under shared care.
Routine referrals
Please send your referral either in writing or by fax to the contacts at the top of this page and address it to one of our adult congenital heart disease consultants, also listed at the top of this page.

Referrals from other hospitals must include a completed Inter-Provider Minimum Data Set| or ensure that one is sent within 48 hours of referring a patient.
