Making a referral
All referrals should be made via the NHS e-Referral service or by typed letter to the address above. Referrals will be reviewed by the team and passed to the most appropriate heart failure service.
Inherited cardiac conditions clinics are not on Choose and Book and should be referred by a typed letter to Dr Carr-White or Dr Ronak Rajani at the following address:

All the clinics will have access to services such as palliative care, psychological support and cardiac rehabilitation (including exercise).
