You’re invited to join our upcoming seminar on improving the health of our population, with a focus on the role and development of integrated neighbourhood teams.
During the session, we'll look at:
- how integrated neighbourhood teams can support better local care
- ways to improve population health outcomes
- working together across health, social care, and community partners
- opportunities for shared learning and next steps for local delivery
We would like to hear your views and questions as we explore how working at neighbourhood level can make a real difference for the communities we serve.
