Community services
We provide community health services across Lambeth and Southwark, allowing us to deliver seamless care for our patients.
We work in partnership with colleagues within the health economy and with the local authorities and voluntary/community groups to provide holistic care.
We deliver services in a variety of locations, including in GP practices, health centres, schools, community buildings and in patients' homes.
Our services range from health promotion through to delivery of high quality care for complex patients in the community centres.
