Double awards success for diabetes project

An innovative project helping people with diabetes in Lambeth and Southwark to live healthier lives has won two prestigious awards.  

The Quality in Care Diabetes Awards recognise good practice by health professionals who work with patients and together to improve patient care.

The Diabetes Modernisation Initiative won in the following two categories:

Best clinical commissioning group/local health board/network initiative

Over the last two years, care for people with diabetes in Lambeth and Southwark has improved significantly. This scheme improved collaborative working between clinical staff and patients, systems and processes of care, and improved the quality of diabetes care in GP practices.

Guy’s and St Thomas’ community diabetes team supported local GPs to identify patients who need extra support to manage their diabetes.

Jennifer Sharpe, who runs the community diabetes service in Southwark says: “By working together we are improving the way we care for people with diabetes locally. We do this by helping patients to self-manage and adjust their medication.”

The judges said: “We loved that this initiative was based on common sense. It’s providing practical, down to earth tools at a relatively low cost and would be very easy to replicate in other areas of the country.”

Best cross-organisational partnership reaching out beyond the traditional team

A group including patients, GPs, specialist doctors and podiatrists (foot health specialists) worked together to improve foot health services in Lambeth and Southwark.

Now patients receive consistent advice and information, and seamless quality care as they move between hospital, community clinics and home. Almost 3,000 extra patients have received an annual foot check since the project began in 2011.

The judges said: “This is a fantastic approach to improve a much needed service and has had a dramatic effect in reducing foot amputations.”
