Neuro rehab team celebrates first birthday

A service that helps patients with brain injuries and neurological disorders has celebrated its first anniversary at an event attended by patients and staff.
 
In its first 12 months Guy's and St Thomas' neuro-rehabilitation enhanced transition team helped 70 patients recuperate at home rather than having to stay in hospital or a specialist rehabilitation unit.

Neuro-navigators (specialist neuro-rehabilitation therapists) work with ward staff to reduce the amount of time patients stay in hospital and refer them to the neuro-rehabilitation enhanced transition team, which includes therapists, rehabilitation assistants and a neuropsychologist.

The neuro-rehabilitation enhanced transition team sees patients with a range of neurological problems – half of patients have brain injuries and 10% have a brain tumour.

They provide an intensive neurological rehabilitation programme and psychological support for up to 12 weeks.

Alexia Winkworth, clinical pathway manager for the team, says: "Our neuro-navigators identify suitable patients for our service who are ready to make the transition from hospital to home.

“Our aim is to provide co-ordinated and joined-up care with our patients and their families at the centre.

"Having a neuropsychologist in our community team is unique. He helps prepare our patients in readiness for rehabilitation and supports and guide our therapists so we all work jointly together as one team”.
