In response to the coronavirus (COVID-19) outbreak, please read our advice and information before sending any referrals.
For new referrals including eReferrals (Choose and Book) please call 020 7188 9910 (9am-3.45pm, Monday - Friday).
If not answered directly, this line will divert to voicemail which is checked regularly during the day.
Email: firstname.lastname@example.org – this email is checked three times per day.
How to refer a patient
We accept referrals from GPs within Lambeth and Southwark.
Please provide clear clinical information to explain your referral, and state the clinical question you want answered.
If you have an opinion as to which service the patient should attend, please provide a clinical explanation for your opinion.
NHS e-Referral Service
Referrals are accepted through the NHS e-Referral System (e-RS).
Please make sure our Southwark GP referral form (Word 48Kb) or Lambeth GP referral form (PDF 92Kb) is attached to your referral within 24 hours to avoid any delay.
Southwark referrals only
Please complete and email our Southwark GP referral form (Word 48Kb) to email@example.com.
Please fill in the referral form as completely as possible to help us to ensure the most efficient service for your patient.
We will triage the referral forms based on the information provided to either MCATTS, orthopaedics or rheumatology assessment clinics, or we may refer the patient for physiotherapy treatment.
Referrals can also be sent in the post but please be aware this could cause a delay for the patient.
Reasonable adjustments for patients
We are committed to providing reasonable adjustments for all of our patients so that they have a positive experience using our services. The details of the adjustments we provide are listed on our e-referral service description.
If you think that your patient could benefit from or might require any of the below adjustments, please can you specify clearly what is needed on the patient’s referral.
- Longer appointments (eg double appointment).
- Timing of appointment (beginning or end of day).
- Environment of appointment eg is there a quiet room available rather than the busy department?
- Carers to come with patient to appointment.
