Referrals

Making a referral

We accept referrals from GPs and other hospital doctors for NHS funded IVF. NHS funding may be available for patients who meet the specific criteria of their local clinical commissioning groups (CCGs). Each CCG has set criteria and will fund different levels of treatment.

It usually takes up to 6 weeks for the funding application process.

All people referred by a GP or hospital will be assessed for NHS funding once an initial registration questionnaire has been returned. We'll apply for funding on their behalf and will complete the documents needed as part of the application.

The number of cycles funded is different for each CCG. Usually 1 to 3 cycle treatments will be funded on the NHS.

Non-pre implantation genetic diagnosis (PGD) fertility

The infertility referral form can be completed by GPs, consultants or the patient, if self-referring.

We also need the welfare of the child and consent to disclosure forms completed by both partners.
