Coronavirus: neuro-rehabilitation community service update

Due to increased demand on the hospital due to the coronavirus (COVID-19) pandemic we are having to make significant changes to our services.

Patients

We are operating a revised community neurological rehabilitation service, starting from Wednesday 1 April.

All referrals for stroke, neurological conditions and the neuro-rehabilitation enhanced transition team (NETT) will now go through a single point of access and be directed to one of two services.

  • Stroke team (for all new organic stroke diagnoses within the last six months).
  • Neuro-rehabilitation team (for all other neurological diagnoses).

We will be contacting our main referrers at St Thomas' Hospital and King's College Hospital.

In order to reduce the possible spread of coronavirus (COVID-19), we will be reducing our non-essential face-to-face contact in the community, and using telephone and/or teleconferencing where able.

Our services will focus on discharging patients home from hospital rather than providing intensive rehabilitation.

Lambeth and Southwark community neuro-rehabilitation referrals (stroke and non-stroke)

We will be contacting our main referrers at St Thomas' Hospital and King's College Hospital.

For all referrals, please complete the referral form (Word 50Kb) and email to gst-tr.neurorehabservice@nhs.net.

Referrals will be triaged and prioritised with a focus on:

  • work to speed up hospital discharges
  • checking to make sure the patient is safe
  • work to reduce a package of care where appropriate
  • optimising patients with a rehab programme and advice to support recovery
  • promotion of self-management techniques
  • signposting and discharge.

South East London Neuro Navigation Service (SELNNS)

The SELNNS covers patients registered with a GP or residing in the south east London boroughs of Southwark, Lambeth, Lewisham, Bromley, Bexley and Greenwich.

Changes to our service

  • We will not be routinely contacting neuro-rehabilitation units.
  • We will not be routinely carrying out community visits. Instead we can arrange to follow up and discuss any concerns or queries on the telephone or video call.

As neuro navigators, we can help to support people to link back into specialist neuro-rehabilitation services, and signpost to local services, once the services are re-established.

Our remit is around an individual’s neuro-rehabilitation needs. We are not able to help with:

  • equipment provision
  • medical/medication enquiries (please contact your GP).

Level 2B neuro-rehabilitation beds

The level 2B neuro-rehabilitation beds are part of the south east London neuro-rehabilitation service.

They are consultant-led and offer specialist intensive rehabilitation for patients within south east London, with a new onset or episode of a neurological injury or condition.

During this coronavirus (COVID-19) response, it is expected that there will be shorter admission periods with a focus on facilitating safe discharges, a log will be kept of patients where more intense therapy input would have been expected and these will be followed up to ensure patients rehabilitation needs have been addressed. We will follow national guidelines that are currently in place for recording outcome measures and data collection.

Referrals for this service should be via Badgernet if the referrer has access, otherwise the referral form (Word 50Kb) can be emailed to kch-tr.selnsreferrals@nhs.net.

There is a new single phone line for the team to enhance handover procedure. This will be monitored throughout the day. The number is 020 3049 7157.

GPs

We are doing everything we can to support GPs and your patients during the coronavirus (COVID-19) pandemic.

If you need advice from our clinical team, please email gst-tr.neurorehabservice@nhs.net. We would encourage you to email leaving a contact number, and a clinician will endeavour to return your call.

We are vetting all referrals. We will be contacting urgent patients but are not currently booking routine patients and so there may be a significant delay before seeing your patient.

Please consider this before making any referral as our capacity to see new patients is very limited.

Frequently asked questions

  • I have been referred but have not heard anything

    If your referral has been emailed to gst-tr.neurorehabservice@nhs.net, we will have received it.

    We are currently not scheduling any new patients unless clinically urgent.

    You will remain on our waiting list and we will be in touch with you in due course.

  • I am worried I might have coronavirus (COVID-19)

    Please call NHS 111 for advice.

  • My medication is running out and I need a prescription

    If the medication is usually prescribed by your GP please contact your GP surgery.
