How to get an appointment

You can be referred by a healthcare professional from:

• Guy's and St Thomas' NHS Foundation Trust
• King's College Hospital
• South London and Maudsley Hospitals NHS Foundation Trust

During your appointment

We'll visit you in the ward or department and assess you. During this assessment, we'll discuss how to improve your health and housing issues.

After your appointment

We'll continue to visit you during your hospital stay.

After you have been discharged, we can arrange a follow-up telephone appointment.
