In response to the coronavirus (COVID-19) outbreak, please read our advice and information before sending any referrals.


To refer a patient

The information on this page explains how to refer a patient to our service. You can request a referral to our community team or outpatient clinic.

Community palliative care team

Referral criteria

Patients with serious life-limiting or life-threatening illness who require support with:

  • pain/symptom management
  • advance care planning
  • meeting their own psycho-social needs and those of their family or partner
  • care at the end of life
  • bereavement support

How to refer

This email account is checked at intervals during the day, and is for referrals only, not general communication.

The information will only be secure when sent from an nhs.net email address. Referrals are assessed according to urgency and priority. If your referral is urgent, please call the department on 020 7188 4754.

Outpatient clinic

Referral criteria

Patients with serious life-limiting or life-threatening illness who require support with:

  • pain/symptom management

How to refer

Referrals may be sent by an EPR outpatient referral form or letter to a consultant.

This clinic is for those well enough to regularly attend as outpatients at Guy's Hospital.

The average wait for a new patient to be seen is 3 to 4 weeks.

This clinic does not involve community or out of hours support for patients.
