To refer a patient

The information on this page explains how to refer a patient to our service. You can specify a referral to our hospital or community teams.

  • Referral criteria

    Patients with serious life-limiting or life-threatening illness who require support with:

    • pain/symptom management
    • advance care planning
    • meeting the psycho-social needs of the patient and their family and/or significant other
    • care at the end of life.

    Patients will be discharged from our service if the above needs are addressed and our input is no longer required.

  • How to refer

    To refer to our teams, please complete our palliative care referral form (Word 148Kb) in full, with relevant supporting information.

    Please note this email account is checked at intervals during the day, and is for referrals only, not general communication. The information will only be secure when sent from an NHS.net email address. Referrals will be assessed according to urgency and priority. If your referral is urgent, please call the department on 020 7188 4754. All two week wait referrals must be submitted using the NHS e-Referral service (formerly Choose and Book).
