In response to the coronavirus (COVID-19) outbreak, please read our advice and information before sending any referrals.


To refer a patient

The information on this page explains how to refer a patient to our service. You can request a referral to our hospital or community teams and our outpatient service.

  • Referral criteria

    Patients with serious life-limiting or life-threatening illness who require support with:

    • pain/symptom management
    • advance care planning
    • meeting the psycho-social needs of the patient and their family and/or significant other
    • care at the end of life
    • bereavement support.

    Patients will be discharged from our service if the above needs are addressed and our input is no longer required.

  • How to refer

    To refer to our teams, please complete our palliative care referral form (Word 148Kb) in full, with relevant supporting information.
