This is a telephone assessment clinic. However, if suitable there are face-to-face appointments available on Friday mornings (times will be arranged individually) to answer a questionnaire and undergo a walking test.

Your GP or other health professional can refer you for an appointment.

Face-to-face exercise and education class or home-based exercise programme.

We'll book you onto the most suitable programme after a telephone assessment.
