Overview
Cardiovascular rehabilitation
We provide a service for patients who have recently had a cardiovascular event. This service is for Southwark or Lambeth residents.
We see patients on the ward following a heart attack. We provide recovery advice and guidance.
We contact local patients once home to monitor recovery, and to continue offering advice and support.
Following a multidisciplinary team assessment, and when appropriate, patients may begin an 8 week recovery programme. This may be face-to-face, remote, or virtual.
The aim of this programme is to help you manage your condition and increase your physical activity. It includes information and discussion sessions, as well as relaxation and exercises classes. It's an opportunity to get support and learn how to look after your health in the future.
You'll be supported by:
- nurses
- doctors
- physiotherapists
- occupational therapists
We'll refer you to other services in our hospital if you need it, such as talking therapies, smoking cessation, dieticians etc.
