How do I get an appointment?
Ask your GP to refer you. You can attend one of our clinics, or we can come to your home or any other location where it is appropriate.

Changing or cancelling your appointment
Please telephone the team where you have your appointment as soon as possible using the contacts at the top of this page.

Before your appointment
You don't have to bring anything with you to your appointment, however if you have any letters relating to your care or any medication you are taking, it would be useful to bring these with you.

What to expect at your appointment
Your first appointment can take up to an hour. You will be a seen by a nurse who will ask you some questions about your general health, and will agree a care plan with you. The nurse will also ask you how you would like to be contacted, for example by text, telephone, letter or follow-up appointment.
Sometimes we may have student nurses present at your consultation but we will ask for your consent for them to be present. If you would like to have someone with you during your consultation, please tell a member of the team. You can bring a friend or relative.
We will not tell other people about your visit. It is up to you to tell them if you wish. This applies to everyone, including people younger than 16 years of age.
We will only pass on confidential information without your permission if we need to protect you or someone else from very serious harm. We will always discuss this with you first.
If you want to know more about confidentiality, please ask one of the clinic staff.

After your appointment
If you have had tests, we will contact you with the results within one week. You will also be given follow-up appointments. Alternatively, you can also contact the person you saw using the contacts at the top of this page.
