You can be referred to our service by your GP or specialist doctor.

If you do not attend your appointment once, we'll re-book it for you.

If you do not attend a second time, we may discharge you from our service.

Before your appointment

For face-to-face appointments, please arrive no more than 30 minutes early.

Please sign up to MyChart before your appointment. We may send you a link with an online questionnaire. Please complete this before your appointment.

For people receiving botox and calcitonin gene-related peptide (CGRP) antibody injections, please email your headache diary to us every 3 months at [email protected].

We need to receive your diaries at least 1 week before your appointment.
