The Lambeth diabetes intermediate care team provides diabetes care to people in the community.
Our aim is to empower you to self-manage your diabetes.
We also:
- support Lambeth GP practices to provide high quality diabetes care
- work with other services including community nursing and secondary care diabetes services
The services we provide include:
- 5 specialist clinics every week across Lambeth
- early kidney disease in type 2 diabetes clinic
- telephone and email advice and support to primary and community care teams
- telephone and email support to people with diabetes attending our service
- home visits to people who are housebound
- virtual diabetes clinics in GP practices
- a diabetes education and self management for ongoing and newly diagnosed (Desmond) programme
- food and diabetes sessions for people with type 2 diabetes
- insulin and GLP1 start groups
- STEPS (type 2 diabetes prevention programme)
