The Lambeth diabetes intermediate care team provides diabetes care to people in the community.

Our aim is to empower you to self-manage your diabetes.

We also:

  • support Lambeth GP practices to provide high quality diabetes care
  • work with other services including community nursing and secondary care diabetes services

The services we provide include:

  • 5 specialist clinics every week across Lambeth
  • early kidney disease in type 2 diabetes clinic
  • telephone and email advice and support to primary and community care teams
  • telephone and email support to people with diabetes attending our service
  • home visits to people who are housebound
  • virtual diabetes clinics in GP practices
  • a diabetes education and self management for ongoing and newly diagnosed (Desmond) programme
  • food and diabetes sessions for people with type 2 diabetes
  • insulin and GLP1 start groups
  • STEPS (type 2 diabetes prevention programme)
