A single referral into a range of healthy lifestyle interventions 

The Lambeth Early Intervention and Prevention Service (LEIPS) aims to improve the quality of life for adults aged 18 years and over who are registered with a Lambeth GP. 

Healthy lifestyle interventions

We provide a range of services to help you:

  • stop smoking
  • reduce your alcohol consumption
  • become more physically active
  • eat more healthily
  • manage your weight
  • complete the Expert Patients Programme to improve your quality of life.

How to make a referral

Lambeth GPs can refer patients via Choose and Book. This allows patients to choose a convenient date and time to be telephoned in order to discuss the services that are available to them.

Referrals can also be made via the LEIPS referral form (Word 100Kb). Healthcare professionals are encouraged to contact us to discuss eligibility criteria and to obtain a copy of the referral form.

All referrals will be contacted within three working days.
