Referrals
1-year programme in south east London (tier 3)
We accept referrals from GPs, practice nurses and other healthcare professionals for patients who:
- are over 18
- are registered with a GP in Bexley, Bromley, Lambeth, Lewisham or Southwark
GP and practice nurses referrals
All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.
You need to select:
- 'Dietetics' as specialty
- 'Weight Management' as clinic type
- 'SEL Tier 3 Healthy Weight Programme' as service name
The referral form is on EMIS or DXS. It is called 'SEL Healthy Weight Management Programme Referral form'.
Your referral must include:
- recent blood test results
- blood pressure
- BMI
- height
Please check the referral form to make sure you've included all the information we need. We'll reject referrals without all the required information.

Body mass index (BMI) requirements
If your patient has no co-morbidities they must have a BMI above 40 if they're white or 37.5 if they're of another ethnicity.
If your patient has been recently diagnosed with type 2 diabetes they must have a BMI above 30 if they're white or 27.5 if they're of another ethnicity.
If your patient has longstanding diabetes or at least 1 co-morbidity associated with obesity they must have a BMI above 35 if they're white or 32.5 if they're of another ethnicity.
Your patient needs to have access to the internet using a device such as a laptop or iPad. It must have a video and microphone.
We don't accept referrals for patients who:
- are pregnant
- previously failed to complete a tier 3 weight management programme
- are having cancer treatment, palliative or end-of-life care
12-week programme in Southwark (tier 2)
We accept referrals from GPs, practice nurses and other healthcare professionals for patients who:
- are over 18
- have a BMI of 30 or higher
- are registered with a GP in Southwark
Your patient needs to have access to the internet using a device such as a laptop or iPad. It must have a video and microphone so they can take part in online group sessions.
We don't accept referrals for patients who:
- are pregnant
- have previously failed to complete a tier 2 weight management programme
- are receiving palliative or end-of-life care
The referral form is available on EMIS or DXS. It is called 'Southwark Tier 2 referral form'.

12-week programme in Lambeth (tier 2)
Local early intervention and prevention services (LEIPS)
We accept referrals from GPs, practice nurses and other healthcare professionals for patients who:
- are over 18
- are registered with a GP in Lambeth
- have a BMI of 30 or higher and 1 or more obesity related co-morbidity
We'll also accept a referral if:
- a patient has a BMI of 27.5 or higher, is from a black, asian or minority ethnic group and has one or more obesity related co-morbidity
- a patient has a BMI of 35 or higher, even if they have no obesity-related co-morbidities
