The @home service
Our @home service provides a range of care to support you in your own home.
We aim to help people living in Lambeth and Southwark:
- return home sooner
- avoid going into hospital
We provide hospital level care in your home.
Our service has a health and social care team who give medical, therapy or social care depending on your needs.
Depending on your needs, we aim to see you within 2 hours of referral, the same day or the next day.
The conditions we care for include:
- heart failure
- pain management
- dehydration
- falls
- acute kidney injury
- viral illness
- infection
- chronic obstructive pulmonary disease (COPD)
- unstable diabetes
- changes in function or mobility
We also provide end of life care.
