Learning disability community health team
Our specialist health team supports adults with learning disabilities, their families and carers.
We do this when their needs cannot be met by mainstream services, even when reasonable adjustments are made.
We see adults 18 years of age or older and living in Lewisham, Lambeth and Southwark and registered with a GP in these boroughs.
Our team is made up of different health professionals who work together to provide the best care for you.
We provide:
- occupational therapy – helping you to perform daily activities, develop skills and take part in activities
- physiotherapy – helping you manage your health condition and physical wellbeing
- audiology (hearing) services – testing how well you can hear and help you keep your ears healthy
- speech and language therapy – helping you communicate and eat and drink more safely
- community learning disability nurses – helping you understand your health condition, manage your condition and get the care you need to improve your health
Research and clinical trials
Research is vital to improving the care that you receive when you're unwell. You can help improve healthcare by taking part in research studies at our Trust. During your appointment, ask your healthcare professional about research. They'll be happy to tell you about research studies you could be eligible to join.
