Learning disability community health team

Who can use the service

We support people with a learning disability to manage their health conditions and get the care they need. We do this when other services find it hard to meet their needs and reasonable adjustments need to be made.

We can help:

  • you have a better understanding of your health needs
  • with your tests, surgery or treatment
  • manage your own long term conditions

We offer training to teams on a range of health conditions most commonly associated with adults with learning disabilities. We do not offer hands on day to day care. 

We will visit you at your home to provide advice on:

  • how you and your carer can learn more about your health
  • healthy diets
  • how to lose weight and keep fit
  • how to learn more about epilepsy and other health conditions
  • relationships and safe sex
  • medicines and how to take them

How to get an appointment

We see people living in Lewisham, Lambeth and Southwark and registered with a GP in these boroughs.

You can:

  • self-refer to the service, or
  • ask someone to refer you

Changing or cancelling your appointment

During your appointment

Your appointment will be in your home or another place in the community. 

When we arrange the appointment with you, we will discuss what you can expect and what you may need.

After your appointment

We will tell you what the next steps are. This may include a follow up appointment at your home or by telephone.
