How do I get an appointment?
The service is available to all adults with a learning disability living in Lambeth, Southwark and Lewisham.
You can refer yourself directly to the service or ask someone to do it on your behalf, such as a family member or your GP.

Cancelling or changing an appointment
Call us to cancel or change your appointment (or you can ask someone to do it for you).

We're open Monday to Friday, 9am to 5pm.

Before your appointment
Remember to bring your Hospital Passport with you to your appointment.
Let us know if you need an interpreter or information about your care in a different language or format.

At your appointment
You can choose where you would like your appointment to be. It could be at:
- your home
- a day centre
- your workplace.

After your appointment
If you're worried about anything after your appointment, speak to the person who visited you or call us.
We will give you and your carers information about your care throughout your assessment and treatment.
