Community rehabilitation and falls

How to get an appointment

We accept referrals from GPs or other health and social care professionals.

We also accept self-referrals.

Changing or cancelling your appointment

Please contact the relevant service.

Before your appointment

Please bring:

  • any recent hospital or medical reports
  • a list of future hospital appointments
  • details of past operations and illnesses
  • a list of medicines you take

During your appointment

First appointment

Our therapist will assess you. The first part of your assessment will take place by telephone. The call will last up to 30 minutes. This will help us decide if you need a face-to-face appointment.

We might ask you to remove some items of clothing and shoes so that we can examine you.

We'll aim to agree goals and work towards achieving them. This may involve completing daily exercises or practising tasks around the house. The goals we set will be time-limited. We'll regularly review your progress. This will guide how long we work with you.

We're a teaching hospital and sometimes students will be at your appointments to learn. We'll ask you if you're happy for them to be at your appointment. It will not affect your care if you prefer not to have them there.

We'll give you exercises or activities to help you achieve your goals. To gain the most from your treatment, please practice your exercises as instructed.

Follow-up appointments

Follow-up appointments are usually carried out in:

  • your home
  • a gym, or
  • a ward

They can last from 20 minutes to an hour, depending on your needs.

If you're an outpatient or we see you in your home, we'll give you details of any follow-up appointments.

If you're an inpatient, we'll see you on the ward. When we see you will depend on your rehabilitation needs.

After your appointment

We'll produce a discharge report summarising your assessment and treatment. We send this to you and your GP.

If you have any concerns or problems with your therapy plan, please contact your therapist. They'll provide you with their contact details at the first appointment.
