Referrals for our Bexley community cardiology service are handled by a dedicated office who can be contacted at or on 020 7188 8952.

Inter-hospital transfer patients

  • Who can be referred

    Patients can be referred for Inter-hospital transfer to Guy's and St Thomas’ from District General Hospitals within our referring region, including Kent, Surrey and the south-east for urgent treatment.  

    Referrals from other hospitals must include a completed Inter-Provider Minimum Data Set (PDF 104Kb) or ensure that one is sent within 48 hours of referring a patient.

  • How to refer

    Inter-hospital transfer referrals to cardiac services at Guy's and St Thomas’ should be completed using the Teleologic IHT system.

    For guidance on how to use Teleologic and prepare your patient for transfer, please refer to the user guide manuals below. 

    In order to review referrals, it is essential that you send any pre-existing imaging particularly echocardiogram and angiograms to St Thomas’ Hospital via either MEDCON or IEP.

    Please specify on your referral which system you have used to transfer the images.  

    Please also attach a PDF copy of your echocardiogram and angiogram reports and well as any other relevant results.
