Your doctor, nurse or social worker will complete an initial assessment.

We'll receive your details if you need a full assessment.

Before your appointment

We'll send you:

• NHS continuing healthcare patient information leaflet, please read this before your appointment

• decision support tool (DST) form, please complete as we will talk about your comments at your appointment

During your appointment

Your appointment usually lasts about 2 hours.

You'll see a continuing healthcare assessor and a social worker.

Your assessment will take place in your own home, either:

• face-to-face
• by telephone
• by video call

After your appointment

We'll send your assessment to the Lambeth or Southwark clinical commissioning group (CCG).

They'll send you a letter with their decision.
