Coronavirus: Lambeth Supported Discharge team update

Due to increased demand on our service due to the COVID-19 pandemic, we are having to make significant changes to the way we run our service to support as many people as possible.

Patients

You will receive a full assessment to identify your needs, but it may mean you are seen a number of times, over days or weeks, depending on your needs. You may sometimes receive a telephone call instead of a face-to-face visit from us. We plan to make contact with you by phone or in person the same day the referral is made, or the next day.

Please call 020 3049 5391 between 9am-5pm, Monday-Friday. Our phone lines are being answered by non-clinical staff who may pass your call onto an appropriate clinician if required.

Information for GP's

We are doing everything we can to support you and your patients during the COVID-19 pandemic. If you require advice from our clinical teams, please contact us:

Please continue to make referrals through our referral line 020 3049 5751 for patients who require rapid support to prevent admission to hospital.

Advice for hospital and community referrers

We are no longer operating separate Enhanced Rapid Response, Supported Discharge and Reablement Teams in Lambeth. In order to support as many patients as possible we are working together as one service known as Intermediate Care Lambeth, to provide a rapid response within two hours of referral.

We are now accepting all referrals for the service via telephone on 020 3049 5751, 8am-6pm, seven days per week (patients need to be home by 7pm for same day assessment).

Once patient demographics have been taken, you will be transferred to a clinician who will triage your referral and take the details from you over the phone. You will be asked to send an up to date discharge summary and/or medication list. We aim to see your patient within two hours of referral.

We are able to provide:

  • health and social care assessment and intervention in the patient’s home, or their normal place of residence
  • intervention including input from occupational therapists, physiotherapist, nurses, rehabilitation support workers, pharmacy and social care staff, with consultant geriatrician oversight.

We are able to support patients requiring:

  • rapid interventions to remain safe in their home environment and prevent unnecessary hospital admissions. This may include care and equipment provision
  • new or additional care in their home environment following hospital discharge (single and double handed up to four times a day)
  • resettlement following hospital discharge.

Patients requiring short-term rehabilitation in their own homes in order to increase independence and reduce ongoing care needs.

Our services 

The Lambeth Supported Discharge team helps you to leave hospital as early as possible and support you to regain your confidence and quality of life. We provide rehabilitation to help you improve your ability to carry out daily tasks such as:

  • getting on and off a chair or bed
  • walking indoors and outdoors
  • washing, dressing and preparing meals
  • managing your medications.

Any health or social care professional involved in your care can refer you to our service. Our service provides intensive rehabilitation and support for people who have been in hospital for longer than 48 hours.

We can also help you to manage any chronic health conditions and may provide you with special equipment to make performing daily tasks easier and safer. Where relevant, we will help you access other community services.
